We're very glad you're planning to be part of our 2012 team! Please read through this page carefully and print out the appropriate paperwork to hand in at Human Resources within two weeks of receiving your offer letter. If you don't have access to a printer, you may pick up the forms at Human Resources.
Paperwork
Please read through this information carefully, as we must have your completed paperwork on file before you start your employment with us. Some of the forms are mandatory for all, while others are only for those who are not yet 18 years old or who wish to use our employee transportation. 1. Required Forms
All employees must print out and complete both of these forms.
- Direct Deposit Authorization For a checking account, you must submit a cancelled check with this form. For a savings account, you must submit a document from your bank that lists the routing and account numbers. (Sorry, a deposit ticket will not be accepted for either type of account.)
- Handbook Receipt, Season Pass Agreement, Emergency Contacts
- Please be sure to bring your I-9 documents when you drop off your paperwork here at the park. If you have a passport, please bring it as your documentation; otherwise you'll need to bring your driver's license plus either your original birth certificate or Social Security Card. 2. Tax Forms
All employees must turn in the first two forms; only Kentucky residents need to turn in the third; only non-Indiana residents who live in the listed states need to turn in the fourth.
- Federal Tax Withholding (W-4 Form)
- Withholding Exemption (WH-4) Holiday World is located in Spencer County, Indiana
- Withholding Exemption Kentucky residents only
- Residence residents of Kentucky, Michigan, Ohio, Pennsylvania or Wisconsin only
3. Work Permits (for employees who are not yet 18 years old)
These forms are only needed if you are not yet 18 years old on the day you complete these forms. If you are a returning employee and we still have your Work Permit on file, you will not need to complete a new one this year.
- Work Permit for Indiana students attending public or private school
- Work Permit for Indiana home-schooled students
- Out of State Work Permit for employees who are not yet 18 and don't live in Indiana
- Permission to work extended hours
4. Employee Transportation
This form is only needed if you are planning to use this optional service.
